Bridging the Gap to Better Health

CHWS SPECIALIZE IN educating community members about preventive health measures and helping them fi nd the services they need.

“The goal of the CHWs is to bridge any gaps that make it hard for patients to access health care,” says Teresa Haatvedt,BSN, health care career and outreach director at HRMC. “These community liaisons build relationships with and connect patients to services in the community.”

HELPING WITH HEART

After referral from a medical provider, patients meet with a CHW to discuss their medical and social needs. CHWs can assist with a variety of challenges, from navigating insurance or medication payment assistance to accessing appointments.

“The CHW program has enhanced our ability to meet our patients where they are,” says Erick Larson, HRMC president and CEO. “Our CHWs are doing magnifi cent work to support a safe and meaningful health journey, no matter where patients have come from or the language they speak.”
