WHAT IS A COMMUNITY LIAISON?

A community liaison is a trusted community member who serves as a bridge between the community and health care services. The liaison will work to build relationships and connect patients with health and services in the community.

HOW CAN A COMMUNITY LIAISON HELP?

Encourages regular check-ups and preventative health measures, such as vaccinations
Able to organize health screenings at the workplace, such as blood pressure, blood sugar, etc.
Provides information that promotes a healthy lifestyle such as diet and exercise
Offers assistance for affordable prescription medication
Advocates for clients who are unable to communicate due to language barriers
Supports members of the community with acute and chronic health conditions

Sau Mai Ramos	“I have a strong desire to make a difference in people’s lives. As a community liaison, I will be able to fulfill my passion of making a positive impact on individual lives on a daily basis. I will be able to fulfill my nurturing and caring desires of helping and educating others.” BACKGROUND Originally from Humacao, Puerto Rico Bachelor's degree in nursing from Interamerican University of Puerto Rico and a Community Health Worker Certificate from Lake Area Technical College Fluent in Spanish and English Began her role as a community liaison in 2022
Saw Kee	“As a community liaison, I want to educate the community members about the health sector, and what they need for their health, especially to support their needs mentally and physically. I aim to help them find the right resources and to promote the importance of health."
