    My Patient Record

    Thank you for choosing HRMC as your provider of medical services. We are pleased to offer secure electronic access to your medical information. Easy access to your health information helps you to be organized and informed… for better health.

    My HRMC Patient Record - Log In



    With My Patient Record, you can get a summary of your:

    • Test Results
    • Medications
    • Health Issues
    • Allergies
    • Immunizations

    Having this electronic access will allow you the ability to review parts of your medical record whenever and wherever you have access to the Internet. You can also download your confidential medical record to your private computer as well as electronically share your record with other medical professionals of your choice if there is a need. Log in to your My Patient Record.

    How to Register

    STEP 1:
    Upon registration you will be asked for your email address. Using this address, an email invitation will be sent which allows you to create your patient portal account. Click on the link provided in the email to start the registration process.

    STEP 2:
    Enter first name, last name and date of birth. The Profile # and email will be automatically populated from the link provided in the email. Next create a user ID and a password. Select the checkbox verifying the information is correct and then click REGISTER.

    STEP 3:
    Select three security questions and provide the answers. These are needed in case you forget your password.

    STEP 4:
    Select the Main Menu button

    STEP 5:
    View Clinical Information

    STEP 6:
    Select the patient to view (if you have completed the Authorization for Disclosure of Health Information form and are an authorized representative for more than one account, you will see them all listed here). This form is available online or in the Health Information Management department of the hospital.

    STEP 7:
    Select the applicable account. All accounts for your visits to HRMC will be listed.

    STEP 8:
    Patient summary information will display. This will include information from your admission to the hospital such as test results, medications, allergies, immunizations and health issues.

    STEP 9:
    To view another account, select “Clinical Information.”

    Frequently Asked Questions

    What is the Patient Portal?
    The Huron Regional Medical Center Patient Portal is an online health electronic document management tool that includes the following clinical data from your Electronic Medical Record (EMR).

    • Test Results
    • Medications
    • Health Issues
    • Allergies
    • Immunizations

    How do I access the Patient Portal once I have completed registration?
    For future visits to the HRMC Patient Portal after the initial setup, go to the HRMC website (www.huronregional.org) and click on “My Patient Portal.”

    How do I set up an account?
    Step-by-step instructions on how to set up an account are included on a handout you may receive from admissions or from staff at the time of discharge. Instructions may also be accessed on the left side of the HRMC website in the “My Patient Portal.”

    Will I receive emails after each admission to the hospital?
    Once the initial email has been sent, the patient or authorized representative will not be sent new emails with each new visit. After each admission to the hospital a new summary of care document will post to your patient portal. You may access the document any time after you are discharged.

    Can I view a family member’s health record in the Portal?
    Yes once they have given permission. Complete an Authorization for Disclosure of Health Information form and return it to HRMC’s Health Information Management department.

    What happens when a child turns 18?
    Your email address along with other demographic information is verified at each admission to provide an opportunity for you to update. On the first hospital admission following your 18th birthday, the email address on your account should be changed from your parent’s address to your own.

    Why are certain test results and health issues not shared in the Patient Portal?
    Your provider may restrict sensitive information pending consultation with you.

    Is my medical information secure?
    We take great care to ensure your health information is kept private and secure. The Patient Portal uses technology to automatically encrypt your session and keep unauthorized persons from reading communications, information, or attachments. Accounts cannot be accessed without your password, therefore you are responsible to protect and secure this information.

    What if I have questions about my medical records?
    If you have questions about your medical record or feel that an error has been made, please contact the HRMC Health Information Management department Monday through Friday, 8:00 a.m.-4:30 p.m. or call 605-353-6228.

    Can I change my password?
    Yes, this functionality can be found under My Options.

    What if I forget my password?
    If you have forgotten your password or your account becomes locked simply follow these steps:

    • Open the forgotten password window.
    • Enter your user name and click “submit.” (Clicking “cancel” will return you to the login window.)
    • You will receive a message stating that an email has been sent to the address associated with your Patient Portal account
    • Check your email for the password reset message
    • Click the link in the email.

    NOTE: If the link is not used in the time frame specified, your account will remain locked and you will have to start the password reset process again. The link will direct you to a screen where you will enter your user name and answer the security questions you selected in the initial registration. Once authenticated, you may enter a new password and confirm it.

    Can I print my information?
    Yes. Click at the top of the page under the patient name “Download (account number) Summary of Care and choose to open the file. Pick the HTML or XML document and when it comes up, click on the printer.

    I was logged out of the portal, what happened?
    If your keyboard remains idle for 30 minutes or more, you will be automatically logged out of the portal. To protect your privacy we recommend you log out of your session if you need to leave your computer for even a short period of time.

    What is your Privacy Policy?
    HRMC Patient Portal is owned and operated by Computer Programs and Systems, Inc. and is fully compliant with federal and state laws pertaining to your privacy. Your name and email address will be treated with the same care and privacy given your health records. View HRMC’s Notice of Privacy Practices under Your Privacy.

    How do I log out?
    Clicking on “sign out” will log you out of the Patient Portal and return you to the login window.

    Technical Problems?
    If technical problems persist, email it@huronregional.org. Please include your full name and a daytime phone number where you can be reached.
