    HRMC offers a very competitive benefits package including:

    • a choice of medical, life, dental, and/or vision insurance
    • generous vacation and sick time
    • generous employer contributions to the 401(k) retirement savings plan
    • tuition reimbursement
    • and many other supplemental benefits

    These benefits are available to employees scheduled to work 40 hours or more during a two-week pay period.

    Health Insurance
    Plan: Sanford Health Plan
    Benefit:  $1,000/$2,000 deductible plans for single, employee + 1 and family
    Eligible: Employees regularly scheduled to work 40 hours or more per pay period
    Effective: The first of the month following 30 days of employment

    HRMC and employee share the premium cots

    Life Insurance
    Plan:
    UNUM Group Life/AD&D
    Benefit: One times yearly salary to maximum of $200,000
    Eligible: Employees working 40 hours or more per pay period
    Effective: The first of the month following 90 days of employment
    Elimination period: 9 months
    Age reduction: 65 percent at age 70, 50 percent at age 75
    Portability option at time employment ends

    HRMC pays 100 percent of the premium

    Additional Life Insurance
    Plan: UNUM Voluntary Life/AD&D
    Benefit: Lesser of 5 times salary or $500,000
    Increments of:

    • $10,000 for employees
    • $ 5,000 for spouse
    • $ 2,000 for child

    Guarantee issue: Employee - $150,000, Spouse - $25,000; Child - $10,000
    Eligible: Employees working 40 hours or more per pay period
    Effective: The first of the month following 90 days of employment
    Elimination period: 9 months
    Age reduction: 65 percent at age 70, 50 percent at age 75
    Portability option at time employment ends

    Employee pays 100 percent of the premium based on age

    Long Term Disability Insurance
    Plan: UNUM Long Term Disability
    Benefit: 50 percent of monthly earnings to a maximum of $5,000
    Eligible: Employees working 40 hours or more per pay period
    Effective: The first of the month following 30 days of employment
    Elimination period: 90 days
    Definition of disability:  2 year own occupation

    Employee pays 100 percent of the premium based on age and salary

    Dental Insurance
    Plan: Delta Dental
    Benefit: Annual max of $1,000 per person per covered year. 
    Eligible: Employees working 40 hours or more per pay period
    Effective: The first of the month following one year of employment

    Employer pays 100 percent of the employee premium--employer and employee share cost of family coverage

    Vision Insurance
    Plan: VSP
    Benefit: WellVision exam: $10 copay for exam & glasses every calendar year. Lenses and contact allowance every calendar year. Frame allowance every other calendar year.
    Eligible: Employees working 40 hours or more per pay period
    Effective: The first of the month following 30 days of employment

    Employee pays 100 percent of the premium--Employee, employee + one, employee + children and family options

    Vision Benefit
    Plan: Vision Benefit
    Benefit: Up to $65.00 every 24 months for non-reimbursed cost of eye exam for either employee, spouse or dependent
    Eligible: Employees working 40 hours or more per pay period
    Effective: Following one year of employment

    HRMC pays upon receipt of completed vision claim form, payment receipt from provider and applicable EOB insurance statement 

    Tax Deferred Annuity (TDA)
    Plan:403(b) Mass Mutual
    Benefit: Traditional and ROTH Tax Sheltered Annuity Plan
    Eligible: Employees with regularly scheduled hours

    Employee contributes to individual plans up to specified IRS limit

    401(k) Retirement
    Plan: 401(k) First Mercantile
    Benefit: Traditional pre-tax and ROTH Employee Retirement Plan
    Eligible: Employees age 21 or older who have worked 1000 hours in a qualifying year.  Quarterly entry dates after eligibility. 

    Employee contributes to individual plans up to specified IRS limit
    Employer contribution declared and paid annually  

    Paid Time Off (PTO) & Extended Illness Time (EIT)
    Eligibility:  Employees regularly scheduled to work 32 hours or more a pay period 
    Accrual: Upon employment
    Usage: After 90 days of employment
    PTO: Paid time off for absences from scheduled work for vacations, holidays, personal business, short term illness, family care, voting and other needs requiring time off from work
    EIT: Paid time for employee absences from scheduled work following 16 hours of PTO due to accident, injury or illness or immediately available for employee time off due to surgery

    Wellness Bonus
    Eligibility:  Employees regularly scheduled to work 32 hours or more a pay period who reach a maximum EIT accrual of 264 hours
    Accrual: Up to 56 hours per pay period based of full-time employment (2.15 hours per pay period) or .02687 per hour paid
    Paid:  50 percent of Wellness Bonus hours are paid the second payroll in November
     

    After payment the account resets to zero and EIT remains at 264 Wellness Bonus is paid first and then EIT hour are used for qualifying illness 

    Bereavement Leave
    Eligibility:  Employees scheduled for work hours and/or scheduled PTO/EIT hours due to death of a family member 
    Paid leave for immediate family members:  Up to 3 days for employee's spouse, child, parent, brother, sister, grandparent, grandchild, father & mother-in-law, son & daughter-in-law, stepparent, stepchildren, stepbrother, and stepsister.  The third day shall be no later than the day after the funeral.
    Paid leave for brother & sister-in-law:  The day of the funeral 

    Shift Differential
    An additional $1.50 per hour is paid to employees working certain evening and night shifts.

    Stand By (On-Call) Pay
    A $1.75 per hour is offered to all hourly employees for time spent on a stand-by status.

    Flexible Spending Accounts
    Benefit: Payment of dependent care and healthcare expense with pre-tax dollars
    Eligibility:  Employees regularly scheduled to work 32 hours or more a pay period are eligible upon employment
    Includes; Dependent care of up to $5,000 annually for qualifying individuals and eligible medical expense reimbursement of $130 minimum up to $2,550 maximum annually
    Flex spending account rollover option: Up to $500 for one year

    Identity Theft Protection
    Benefit: Supplemental identity theft coverage
    Eligibility: Employees regularly scheduled to work 40 hours per pay period
    Effective: First of the month following 30 days of employment

    Employee pays 100%

    Tuition Assistance
    Benefit: Tuition assistance, up to $1,200 per fiscal year for approved courses offered by an accredited educational institution or recognized certification program
    Eligibility:  Employees who have been employed for a minimum of one year and regularly scheduled to work 32 hours per pay period prior to the last six months

    Employee Store
    The employee store offers discounts prices on various food and non-food items to all employees.

    Fitness Center Membership
    Discounted membership rates offered to employees with regularly scheduled hours.  Employee contributes 100 percent.
